You will receive that extra personalised service where your experienced aged care dietitian will provide in home educations, meal plans based on the food stores in your cupboard, supplement recommendations when regular food intake is not sufficient, meal delivery service options, meal preparation demonstrations or family based recipe modifications and much more to ensure optimal health outcomes.
As Dietitians, we recognise that food is a huge and emotive subject for many people, and in particular, older people. It can bring family and friends together, help us unite and reflect on many different areas of life and experiences. Food is the element that is used to celebrate, reflect culture and traditions and affects our health and quality of life. Like many of us, food is a comfort when times are difficult, and when cooking or eating becomes difficult, many older people can experience frailty or poor health. What to expect if I see a Optimum Intake Aged Care Dietitian
As part of the service, individuals will receive a one on one session with a Dietitian in the comfort of their own home, removing the need to arrange transport and travel to an office. During this session the Dietitian will conduct a comprehensive dietary assessment and prepare a comprehensive report. Ongoing sessions, monitoring and supports will be tailored to optimise each individuals nutritional and home care needs. Where appropriate we can liaise with other health professionals involved in your care as well as other family or carer supports. We aim to create a community approach to supporting your independence and ability to remain living in you home and accessing optimal nutrition.
