What is the CHSP?

The CHSP is an entry-level aged care program that provides subsidised support to older adults needing help to stay at home. Through Allied Health – Dietitian Services, we partner with clients to enhance their independence at home, maintain their well-being through food, and work towards achieving their personal health goals. It focuses on wellness and reablement, working with individuals to build on their strengths and abilities, improving their health, independence and sense of purpose.

This program is perfect for older adults who are:

  • 65 years or older and need assistance to live independently.

  • Individualised nutrition care plans to support your specific health and nutrition goals.

Introducing Our Restore and Nourish Programs

Restore Program

Designed for clients needing short-term support to improve function and avoid further decline, the Restore Program provides:

  • 8 occasions of service within 3 months.
  • Focuses on addressing issues like low risk for malnutrition, recent weight loss, and new medical diagnoses.
  • Support for conditions such as dementia, COPD, and gastrointestinal concerns (and more).
  • Interventions include high energy and high protein education, meal delivery support, and food fortification strategies.

Nourish Program

For those requiring more intensive care, the Nourish Program offers:

  • 12 occasions of service within 6 months.
  • Higher-intensity support for moderate to severe malnutrition, significant weight loss, and therapeutic diets.
  • Assistance for chronic conditions like diabetes, cancer cachexia, and wound healing.
  • Interventions include meal plans, diet analysis, and liaising with other health professionals.

  • Meal delivery services to ensure meals meet the nutritional needs of clients who are at risk of malnutrition or already malnourished. Clients at risk of malnutrition need dietitian support to complement meal services, ensuring dietary adequacy.
  • Nutritional supplements for clients requiring additional dietary support beyond food. With our food first approach, we only recommend supplements as necessary, for only as long as necessary, with the aim to return to full food diet.

What Does It Cost?

The CHSP program completely funds our services to you, so you do not have any out-of-pocket expenses. While some providers may charge a small contribution fee, our company remains committed to creating affordable access to dietary care when you need it most.

How to get started:

1. Secure Your CHSP Code

  • To access CHSP dietitian services, you’ll need a CHSP code specifically for allied health services.
  • This code is provided by My Aged Care after an aged care assessment.

  • Once you have your CHSP code, reach out to us via phone, email or through our website.
  • Our friendly administration team will assist you with the referral process and ensure all necessary details are in place to begin providing services.

Once your referral is accepted, our friendly administration team will contact you to discuss your specific needs and complete some pre-visit questions. Your first appointment will be an initial assessment conducted in the comfort of your home by one of our experienced dietitians. We welcome the presence of anyone you wish to have with you during this visit.

Our dietitian will:

  • Evaluate your nutritional needs and health goals.
  • Work collaboratively with you to create a tailored care plan.
  • Discuss our recommended program: either our Nourish or Restore program, determined by your health, needs and goals, including the frequency of follow-up visits and other nutritional services.
