Changes to Aged Care Funding: What You Need to Know

From 1 July 2025, the Support at Home program will replace the existing Home Care Packages (HCP) and Short-Term Restorative Care (STRC) programs.

Support at Home is a coordinated package of care and services designed to meet the ageing-related care needs of eligible older people. The new program introduces eight ongoing classifications, each with increasing levels of funding to provide appropriate support based on a person’s assessed needs.

Under Support at Home, dietitian services will fall under two main areas:

  • Clinical Supports – Allied Health and Other Therapeutic Services: This includes general dietetic care provided in the home, such as nutrition education, therapeutic dietary advice, and chronic disease management.
  • Nutrition – Prescribed Oral or Enteral Nutrition: This refers to prescribed nutrition support products or enteral feeding where food alone is not sufficient or possible, ensuring medical nutrition therapy is accessible to those who need it.

What Is Self-Management in the Support at Home Program?

Self-management allows older adults to have more choice and control over their care under the Support at Home program. It means you can take an active role in decisions about the services you receive, how your budget is used, and which providers or workers deliver your care.

With support from your provider and care partner, you can choose to self-manage all or just some parts of your care—like selecting your preferred allied health professionals or managing your service schedule. This flexible approach recognises that older people, with help from carers or supporters if needed, are best placed to shape care around their own needs and goals.

Third-Party Worker Engagement

At Optimum Intake Dietitians, we’re committed to building strong relationships with Support at Home providers and our local community to ensure older adults receive the highest quality nutrition care.

We understand that many clients are choosing to self-manage their care or work with providers who engage third-party specialists. That’s why our team is fully equipped to provide services as an associated third-party provider under the Clinical Supports – Allied Health and Other Therapeutic Services category of the Support at Home program.

Support at Home providers can engage Optimum Intake Dietitians to deliver in-home nutrition services on an ongoing or ad hoc basis—allowing flexibility to meet the client’s individual goals and support plan.

We meet all regulatory requirements under the Aged Care Act and the Strengthened Aged Care Quality Standards, giving both our clients and the providers we work alongside confidence in our care, compliance, and professionalism.

In addition to the Support at Home funding model available for adults over the age of 65years, Optimum Intake is also an approved provider of Allied Health – Dietitian Services under the Commonwealth Home Support Programme (CHSP). The CHSP is an entry-level aged care program that provides subsidised support to older adults needing help to stay at home.
